  9.      FOR VALUE RECEIVED, the undersigned jointly and severally
  10. promise to pay to the order of _, the sum of _ [$_] Dollars,
  11. together with interest thereon at the rate of _% per annum on
  12. any unpaid balance.
  13.      Said sum, inclusive of interest, shall be paid in _
  14. [monthly/weekly] installments of $_ each, with a first payment

  16. [month/week] thereafter until the full amount of this note and
  17. accrued interest shall be fully paid.  All payments shall be

  22. holder in the event the undersigned shall default in making

  25. pay all reasonable attorney fees and costs of collection to
  26. the extent permitted by law.  This note shall take effect as a
